Incident:
On 29 April 2019, a workers sustained serious crush injuries from being pinned between the offside wheel and the chassis of the HT2 when performing maintenance task. The injured worker was in a coma for 7 days and in hospital for 22 days with injuries including a fractured trachea, scapula and ribs, brain injuries and loss of voice.
Alleged breach:
PCBU having a health and safety duty under the Occupational Health and Safety Act 2004 and in connection to matters relating to alleged contraventions of the OHS Act and Occupational Health and Safety Regulations 2017.
Summary of undertaking:
The following are key commitments given:
- Commissioning and supplying a fully serviceable Mobile Safety Training Trailer to Civil Contractors Federation (CCF) Victoria
- Supplying the necessary training equipment for the MST Trailer to enable CCF Victoria to deliver the CCF Courses
- Commissioning CCF Victoria to develop a case study of the incident for use in future safety courses such as the Mobile Plant Risk Assessment course.
